What happens before the visit?
Practice Support Consultants will phone the practice to make a time and date for a visit.
What happens during practice visits?
During practice visits Practice Support Consultants will
- Provide the practice with a hardcopy of the Division General Practice Resource Guide 2009
- Complete a GP Practice Visit checklist
- Give a verbal overview of the resources and services available from the Bankstown GP Division
- Provide basic orientation and updates regarding patient management software.
- Orient the practice to the Division website
- Liaise with General Practitioners and practice staff to identify and prioritise practice support needs.
- Organise a date for follow up visit.
What are some specific examples of practice support consultancy?
- practice accreditation support
- Practice computerisation eg Medical Director, Best Practice and Argus installation, as well as hardware, office software and Internet configuration
- eHealth, including ARGUS, data extraction and data management support
- Development of Register, recall reminder systems (hybrid and electronic) for chronic conditions and preventable diseases (cervical screening, childhood immunization).
- Chronic Disease Management systems (including team care arrangements, care plans and reviews) including asthma, cardiovascular disease and diabetes
- Preventable diseases initiatives including ATSI health checks, child health checks, 45-49 year old health checks 75 year old health assessments, as well as any other relevant health checks and assessments
- Assistance and advice with use of the most appropriate MBS item numbers
What happens after a practice visit?
- The Practice Support Consultant will enter the details from the GP Checklist into a database
- The Practice Support Consultant will follow up on priorities identified during the visit
