Enhanced Primary Care (EPC) plans

The Australian Government Enhanced Primary Care (EPC) initiative offers a framework for GPs to provide more preventive and systematic care for older Australians and supports the coordination of multidisciplinary care for patients with a chronic condition and complex care needs.

GP Management Plans and Team Care Arrangements for people with chronic conditions

Medicare rebates are available to GPs for preparing and reviewing GP Management Plans for patients with chronic medical conditions. For patients requiring multidisciplinary care, GPs can claim from Medicare for coordinating team care planning and review services.

The MBS items apply for treatment of people with asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke. They complement GP Mental Health Care items for treatment of patients with mental disorders.

Medicare Health Assessments (Items 700 - 719)

Health assessments allow a GP to undertake a more comprehensive assessment of a patient with complex care needs. Health checks also permit the needs of specific groups (Aboriginal and Torres Strait Islander people, refugees and aged care residents) to be addressed in a targeted and culturally appropriate manner. MBS items are available for the following:

•               45 Year Old Health Check

•               Aboriginal and Torres Strait Islander Resource Kit - health checks for children, adults and older persons (55 years+)

•               Comprehensive Medical Assessment for Patients of Aged Care Homes

•               Health Assessments for Older Persons (75+)

•               Intellectual Disability Health Assessment

•               Health Assessment for Refugees and other Humanitarian Entrants

•               Healthy Kids Check

•               Type 2 Diabetes Risk Evaluation
