This page provides forms for the Medicare dental items for people with chronic conditions and complex care needs.Who is eligible for the dental health scheme?
Individuals must have
- a chronic condition that is likely to be present for six months or longer.
- Their oral health must impact on or likely to impact on their general health.
- They must have an EPC plan (a GP Management Plan item 721 AND a team Care Arrangement)
Referral to dentist form for eligible patients
GPs must use the Referral Form for Dental Service Under Medicare issued by the Department of Health and Ageing, or a form that substantially complies with this referral form.
The form can be downloaded from www.health.gov.au/dental or obtained by calling the Department of Health and Ageing on (02) 6289 4297
