Medical software applications currently used in general practice are excellent for data storage and retrieval on a patient by patient basis.
However, most clinical desktop programs have a limited in their capability to retrieve of data for the entire practice patient population.
This makes them generally too difficult to use for systematic management of patients.
Effective data management is important for chronic disease management of the patient population and to claim the PIP ehealth incentive.
Extended Primary Care initiative
The Enhanced Primary Care (EPC) initiative offers a framework for GPs to provide more preventive and systematic care for older Australians and supports the coordination of multidisciplinary care for patients with a chronic condition and complex care needs.
Chronic disease management in general practice involves appropriate prevention, early identification and best practice management strategies. Effective information management that can assist in promoting improved clinical, patient and practice outcomes.

Medicare rebates are available to GPs for preparing and reviewing GP Management Plans for patients with chronic medical conditions. For patients requiring multidisciplinary care, GPs can claim from Medicare for coordinating team care planning and review services.
The CDM items apply for treatment of people with asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke. They complement GP Mental Health Care items for treatment of patients with mental disorders.
