A Home Medicines Review (HMR) is a Government funded, consumer-focused, collaborative service involving both GPs and community pharmacists.
HMR provides an opportunity for the GP to view a “patient-medication relationship” in a context outside the patients visit to the surgery.
How does it work?
The GP generates a referral for a patient to their local pharmacy. A pharmacist conducts an interview, preferably in the consumer’s own home, and then writes a report back to the GP, who then discusses any recommendations with the consumer and may make appropriate changes to their medication regime.
The process (Medicare Item 900) involves two consultations for the GP.
The first consultation identifies potential patients and arrange the procedure.
The second consultation after the pharmacist’s visit will result in a report. It also requires the GP and Pharmacist to discuss the clinical report, where appropriate. A copy of the Medication Management Plan is provided to the patient and the community pharmacy.
What are the benefits of an HMR?
An HMR will provide the GP with comprehensive, up-to-date information about the medicines, complementary products, over-the-counter medicines, devices and other prescriptions being used by their patient.
The patient will receive assistance with medications, aids and devices to improve compliance, and early detection and management of medicine-related problems.
GPs are able to participate in the Home Medicines Review program designed to improve the quality use of medicines by patients in the community.
What can the Division offer?
The Division is actively involved in the delivery of the Home Medicines Review (also previously known as the Domiciliary Medication Management Review) program in partnership with the Pharmacy Guild of Australia.
The main objective of HMR is to improve a patient’s understanding of their medications and thereby optimize benefits and health outcomes.
The HMR program promotes, educates and provides support by facilitating one-on-one visits, articles in the Division’s newsletters and group workshops with GPs, Pharmacists and Community Groups.
